Senior Social Worker-PRIMARY CARE MENTAL HEALTH INTEGRATION (PCMHI)
Primary Care Mental Health Integration (PCMHI) functions as an extension of the PACT Team and serves as the access point to mental health. The Incumbent works within the PCMHI Model serving as a Senior Social Worker/Behavioral Health Consult. He/she works closely with the physician team lead, nurse case manager and all teamlets to provide brief assessment and consultation/referral for veterans with psychosocial concerns covering the full spectrum of disorders.
