ChristianaCare's Office of Health Equity is looking for a Senior Social Worker I/Licensed Clinician to join their team. This position will support the New Castle County Division of Police Behavioral Health Unit, Hero Help Addiction and will serve as a liaison among providers, hospitals, peer counselors, and people seeking treatment for and addressing their substance use disorder. The Senior Social Worker I/Licensed Clinician position is a grant funded position with an expected expiration date of October 2023.
This position requires flexibility with a rotating shift and on-call responsibilities as well as the ability to apply ethical and legal principles to the treatment ofindividuals with substance use disorder. Some Responsibilities include:
Provide quality case management and psychotherapeutic services to patients with co-existing disorders of mental illness and substance use disorder.
Conduct timely screening and assessments to all individuals engaged through non-fatal overdose outreach to determine when a co-existing disorder may be present.
Conduct psychosocial and chemical dependency assessments to document patient history and identify preliminary issues for treatment focus by interviewing the patient and their family members.
Participate in the development and implementation of treatment plans, ensuring that concise, realistic care plans are carried out.
Provide regular follow-up to assigned offenders to determine current mental status and functioning.
Desired candidates need to possess either a Masters of Social Worker or a Master's degree in a recognized mental health field. Possession of a LPC, LMSW or LCSW and CADC/CAADC is required and much have a minimum of four years of experience in the field.
