Join our Team at WakeMed Raleigh as a Behavioral Health Counselor / Transitional Care Specialist (LCSW Required – Full Time)
As the Behavioral Health Counselor – Transitional Care Specialist (TCS), you will provide transitional care services to our patients utilizing emergency and crisis services due to substance use or mental health disorders. The TCS is responsible for screening all referrals and assessing potential patients for appropriateness of transitional care services. You will also be responsible for arranging outpatient care if a different level of outpatient services is required on a case-to-case basis. The TCS will also collaborate with the Peer Support Specialists and facilitate the safe and timely transition from hospital care to community-based services. The Transitional Care team at WakeMed is comprised of two (2) Transitional Care Specialists and three (3) Peer Support Specialists.

3 years of clinical or mental health work and experience with Substance Abuse / Addiction required
Bachelor’s Degree in Nursing OR Master’s Degree in Social Work OR Counseling required
Master’s Degree in Nursing preferred
Licensed Clinical Social Worker required AND/OR Licensed Professional Counselor preferred
Licensed Clinical Addictions Specialist preferred
Vehicle Operator Insurability preferred
Certified Notary preferred
Hours of Work: M – F, 8 hour shifts, flexible
Weekend Requirements: As needed
